CPW is getting ready for another great season!
Paper Work Night has been scheduled for
Sunday, July 13th, from 4-7 pm
in the rear of the Kennedy (100 Dedham St.)
All registered participants should have received
a letter regarding paper work night.
Please remember to bring all necessary paper work
including 2008 Pop Warner Medical Form.
If you did not receive a letter, please email
Walk-in Registration has ended,
but Mail-in and Online Registration will continue.
Forms can still be downloaded and mailed to the above PO Box
Please note that late fees will apply after May 11th.

This form should be filled out if your child is injured and requires medical attention.
This is only for secondary coverage, you must submit your claim to your primary insurance company first.
